HUD hosts NPMA-33 for FHA and VA use when wood-destroying insect information is requested. It is not an official Colorado state report, and the party requesting the inspection should confirm the form it accepts. Outside those counties, the VA appraisal-trigger rule still applies.

Is NPMA-33 a Colorado state form?
